Go back to the separation, please

I have some questions for you.
Why did you separate? And what was the reason for separation as opposed to divorce?
Was there no contact between you two at all before his call to you on 12/04? Did you never contact him?( Usually a separation instead of divorce means that you two are working on issues apart, before making the final decision to split up. Is that what you were doing?)
Why did you decide you get back together? You had taken a legal step, but he never signed the papers, I'm assuming you mean that you had divorce papers drawn up. So again, what was it that convinced you to go back to him?
Without this background, it's pretty hard to give any advice.
