"I feel like I've been reborn, like I've got my life back," he says. "The biggest surprise was how easy it was to change my life."
Thomas then gets up to model his "goal outfit," a suit that he hoped to be able fit back in again one day.
"I can't believe I'm 100 pounds overweight," says Modesto.
Thomas is confident that his friend will succeed. "He's been wanting to do something about his weight and he just needs something to get him rolling," he says.
Dr. Phil asks Modesto if he is serious about wanting to change his life.
"Yes, I am the heaviest I've ever been," says Modesto, who now weighs 273 pounds.
Dr. Bruuner tells Modesto that at his current weight he is eight times more likely to have a heart attack and 16 times more likely to develop diabetes than if he were at his goal weight.
"You are walking time bomb," says Dr. Phil. "But you have no idea how fast you can reverse those trends."
